The $20 fee at the Maricopa County STD Clinic includes testing for syphilis, gonorrhea, chlamydia, and HIV. Between 8 a.m. to 3 p.m., a rapid HIV test is offered with results available in about an hour. A fee of $20 will be requested at all visits.
Note: The clinic does not perform treatment for genital warts, herpes testing, HPV testing, PAP Smears, birth control (other than condoms), general medicine/non-STD problem help, genital herpes testing, HIV treatment, or pregnancy care.

This form can be filled out before your visit to the clinic to speed up the registration process. Please fill it out and bring it with you when you come in.
In order to obtain copies of a medical record or testing results, a picture ID with the name and date of birth matching the medical record must be presented at the time of request. Records releases are done Monday through Friday, 9 a.m. to 11 a.m., and from 2 p.m. to 4 p.m. only.
